You can easily configure these appraisal form templates to meet your requirements. Follow the path:
On the Homepage dashboards, click Product configurations.
Go to Appraisal plans under Performance Management and open an appraisal plan.
There are multiple tabs - Plan introduction, time & notifications, Form & settings, and Visibility rules. Go to each section to make changes in the rules.
Save the changes.
